VBS RELEASE FORM

HEALTH & SAFETY - A completed registration form is required for each child to participate in Maranatha Full Gospel Fellowship VBS. Your signature below indicates your child is in good physical condition and has your (parent’s or guardian’s) permission to participate fully in all activities unless otherwise noted on this form. ASSUMED INHERENT RISKS - There are inherent risks with participating in a VBS program including but not limited to the following: risks associated with the behaviors and actions of programmed activities such as recreational games, playground activities and risk associated with the behaviors and actions of other participants, employees & volunteers and agents of Maranatha Full Gospel Fellowship and other individuals. For information on specific activities, please call Maranatha or speak to one of our hospitality workers. PERMISSION TO PARTICIPATE - I, the legal parent/guardian of the minor listed below, understand there are inherent risks within participating in Maranatha Fellowship’s VBS. I give permission for my minor child to fully participate in Maranatha Full Gospel Fellowship’s VBS for which my child is registered. My permission extends to all of the activities, on and off site, associated with VBS, unless otherwise noted by me on this form. PERMISSION TO TRANSPORT - I, the legal parent/guardian of the minor listed below, give permission for said minor to be transported by approved VBS staff and vehicles for VBS activities and/or emergency medical treatment. PERMISSION TO USE AUDIO, VIDEO AND PHOTOGRAPHIC IMAGES - I, the legal parent/guardian of the minor listed below, give Maranatha Full Gospel Fellowship permission to use audio/video footage and photographic images that include my child at VBS in publications, such as the VBS newsletter, website, posters, slideshow, etc...I understand that the children’s names will not be released. LIABILITY RELEASE - In consideration of the permission granted the herein named minor to participate in VBS activities, I am the parent or legal guardian for minors listed and personal representatives, assigns, heirs, and next of kin (Herein referred to as releasers), hereby releases, hold harmless, waives, discharges, and covenants not to sue or bring any action whatsoever against Maranatha Full Gospel Fellowship, its agents, servants, volunteers, employees (hereby referred to as releases) from all liability to the releasers for all loss or damage and any claim brought on behalf of the child named below or demands on account of injury to the person or property or resulting death of the releasers whether caused by negligence of the releases or otherwise while participating in activities associated with Maranatha Full Gospel Fellowship VBS. While Maranatha Full Gospel Fellowship VBS staff will make every effort to ensure the safety and well-being of your child, we do require that this form be read, filled out, and signed and dated by the parent or legal guardian of anyone under the age of 18 who wishes to participate.
